Boardroom Rental Request - Johnson City/Jonesborough/Washington County Chamber of Commerce

Requests to use the Johnson City Chamber's Eastman Credit Union Boardroom are available under the guidelines noted below. This request form must be completed, submitted and approved. The Chamber reserves the right to accept and/or deny any request for meeting room usage. Once approved, the user must indemnify The Chamber from any damage caused by the user and hold The Chamber, its employees and Board of Directors free from any liability.

I understand that smoking is NOT allowed ANYWHERE inside or outside of the building including the parking lot and terrace. I understand that if I expect smokers to participate in my event, I will explicitly tell them that, like those who work in the building daily, they must leave the premises.

By typing your signature, you acknowledge that you are a duly authorized representative of the above-referenced organization and that you do hereby certify the purpose of this request is properly stated. You do also certify that this purpose will not be disruptive to the normal operations of the Johnson City Chamber of Commerce. You agree to be responsible for any and all charges listed herein, and you also agree to be responsible for any and all damages resulting from this activity. You hereby agree to indemnify and hold the Johnson City Chamber harmless from any all liabilities arising out of the use of the facility including, but not limited to, personal injury, property damage, court costs and attorneys' fees. It should be noted that the Johnson City Chamber will not be responsible for injuries resulting from activities involved with this request.
